PERKESO Disability Management Program

INTRODUCTION TO PERKESO DISABILITY MANAGEMENT PROGRAM

Insured person under PERKESO who experienced physical or cognitive disabilities, whether temporary or permanent, that is due to injury or illness will undergo rehabilitation through PERKESO’s holistic Disability Management Program. This program adopts a biopsychosocial approach, involving referrals for physical or vocational rehabilitation to help individuals achieve optimal independence and the ability to return to work, in ensuring they are capable in generating income for a sustainable living.

ELIGIBILITY UNDER PERKESO DISABILITY MANAGEMENT PROGRAM

Eligibility for PERKESO Insured Persons to participate in the PERKESO Disability Management Program is subject to the approval of claims under the relevant act, supported by a recommendation from the treating doctor confirming that the disability experienced is caused by the injury or illness forming the basis of the claim to PERKESO.

OBJECTIVES OF PERKESO DISABILITY MANAGEMENT PROGRAM

  1. Proactive Rehabilitation Support
    To provide a structured rehabilitation support to PERKESO Insured Persons who experiencing disabilities, enabling them to regain independence in activity daily living (ADL).
  2. Integrated Case Management
    To encourage active participation of PERKESO Insured Persons in rehabilitation through integrated case management, combining various services and expertise holistically. This supports recovery by emphasizing individual potential and abilities through close collaboration among all involved parties and stake holders.
  3. Quick Access to Services
    To expedite access to healthcare, rehabilitation, and return-to-work services, including the provision of assistive devices, dialysis treatment, prosthetics, orthotics, and comprehensive rehabilitation care.
  4. Systematic Case Monitoring
    Systematic monitoring of cases by Disability Management Officers (DMO) /Assistant Disability Management Officers (ADMO) together with Insured Persons, treating parties, families, employers, and service providers to ensure effective and sustainable case management interventions.
  5. Work Ability Assessment (WAA)
    To conduct targeted Work Ability Assessment to identify the actual capabilities of PERKESO Insured Persons in various job contexts and assist employers in determining suitable job accommodation based on current ability levels.
  6. Social Integration
    To support reintegration into the community and labor market, in line with the principles of inclusivity and comprehensive social protection.
