Permohonan Faedah

Buat masa ini sebarang permohonan faedah hendaklah dikemukakan kepada pejabat PERKESO yang berdekatan untuk semakan dan juga pengesahan pegawai kami. Namun, PERKESO akan terus menambahbaik pengalaman pelanggan untuk memudahkan anda berurusan dengan kami. Berikut adalah proses permohonan faedah secara am.

Langkah 1

Muat turun borang-borang yang ditetapkan mengikut jenis faedah yang ingin dimohon (Rujuk di bawah).

Layari Portal Permohonan Faedah untuk membuat notis-notis berikut:

  • Notis Kemalangan
  • Notis Penyakit Khidmat
  • Notis Kematian

Langkah 2

Lengkapkan semua bahagian yang perlu dilengkapkan oleh pemohon dan sediakan dokumen sokongan yang disenaraikan dalam panduan. Bagi Borang 34, sila rujuk kepada panduan pengisian di pautan berikut.

Kemukakan borang-borang serta dokumen sokongan di kaunter pejabat PERKESO yang berdekatan.

Berikut adalah borang-borang yang telah disusun mengikut jenis faedah. Muat turun semua borang-borang tersebut mengikut jenis faedah yang ingin dimohon.

Nota: Tuntutan faedah SPS hendaklah dihantar ke pejabat yang terdekat.

Benefit Application

At the moment, all benefit applications must be submitted to the nearest PERKESO office to be reviewed and verified by our officers. However, PERKESO will continue to improvise the customer experience to make it easier for you to engage with us. The following is the general process for benefit application.

Step 1

Download the given form below based on the type of benefit you want to apply.

Visit Portal Faedah to create an application for the categories below:

  • Accident Notice
  • Occupational Disease Notice
  • Death Notice

Step 2

Complete all sections that the applicant is required to complete and provide the supporting documents listed in the guide. Kindly refer to the guideline below for Borang 34.

Submit the forms and supporting documents at the nearest PERKESO office counter.

The following are the forms that have been arranged according to the type of benefit. Download all the forms according to the type of benefit you want to apply. The forms are only available in Bahasa Malaysia, hence if you have trouble understanding the forms you can contact PERKESO via any available channel or have someone who understands the language to help you complete the forms.

Training Application

EIS provides vocational training to eligible applicants. The training fee of RM4,000 will be covered by EIS. Trainees who attend all their training sessions can also receive financial incentives.
