- Click at the link sub menu at the Download menu in the main SOCSO website.
- For example, click the Checklists and Forms submenu.
- Browse for the forms that you are looking for. Click the Download button at the Forms list.
- Click to start the Download.
- A message box will appear requesting the destination folder for the download.
- Decide on the appropriate destination to save the file. Click Save.
